Authorization for practical exam at the CBR
To request a test or exam for your driver's license, you must first authorize your driving school. We recommend doing this right at the start of your driving lessons. This way, you avoid forgetting and unnecessary delays.
You can easily authorise this via the CBR using your DigiD and our driving school registration number: 8737G1 .

Health declaration at the CBR
The Health Declaration is a questionnaire about your physical and mental health. You need this declaration to obtain a certificate of fitness from the CBR (Central Office for Motor Vehicle Driver Testing).
When you take the driving course, you'll receive a practical manual containing the Health Declaration. For other categories, or if you're changing driving schools and haven't yet taken the exam, we'll provide you with a copy that you'll need to complete and sign.
Please note: you are required to submit the Health Declaration to the CBR (Central Office for Motor Vehicle Driver Testing) yourself. Always do this on time, especially if you answered yes to one or more questions. The Health Declaration is valid for one year.
